Conduct admission interviews and formulate an initial psychosocial assessment. Coordinate with inter-disciplinary team, residents and their designated representatives to provide palliative/comfort care during the last stage of life and guide them in health care decision-making process. Assist residents with adjusting to new environment and changes in medical/physical conditions. Monitor resident’s progress according to measurable goals described in care plans. Complete and update assigned section of the MDS 3.0. Interpret treatment to Chinese-speaking residents and their designated representatives. Coordinate discharge planning, including home care services. Refer resident to community resources, if necessary.
