The Supportive Services for Veteran Families (SSVF) Case Manager provides services to participants in our SSVF Program. The Case Manager acts as a part of a team at the Fairbanks Rescue Mission to provide assistance to veterans and veteran families in our community.

Minimum Qualifications:

Excellent verbal, written and interpersonal communication skills

Ability to work effectively with people of diverse educational and cultural backgrounds

Associates Degree in human services or related field

2 years experience in delivering case management services preferred

Knowledge, Skills and Abilities:

Able to complete all required paper work in a timely, accurate and professional manner

Computer literacy, including knowledge of basic software applications and familiarity with the internet and email communications

Knowledge of veteran related issues and/or the homeless population with preference given to veterans

Must maintain a valid Alaska drivers license

Job Duties:

Provide individual case management sessions with clients at regularly scheduled intervals.

Develop individual housing plans for qualifying participants.

Document all client contact and progress, including time and date, type of contact, outcome and plan of action.

Provide or broker services to holistically address clients needs in a timely and efficient manner to ensure goal accomplishment and procurement of housing and/or sustainable employment. These services include, but are not limited to: assistance obtaining VA benefits; housing referrals; assistance in applying for income assistance, food assistance, childcare and employment services; budgeting classes; education; and medical and mental health referrals.

Satisfactorily complete all required trainings as defined by agency policy and procedures.

Participate in the discharge process and aftercare needs of the client.

Provide after-hours on-call services as assigned.

Oversee coordination of services with other community service providers, including coordination of appointment times, facilitating activities and resolving issues.

May also perform the duties of employment training specialist and/or job developer.

Provide outreach to the community, landlords, agencies, and veterans on the streets.

Provide regular updates to the Program Director.

Other duties as assigned.
