Service Coordinator - Mountain View Senior Apartments

Southeast Senior Services is seeking a Service Coordinator to assist the residents of Mountainview Senior Apartments connect with the community services they need. Job duties include individual case management (including annual Independent Living Needs Assessments and resident education about available services), program coordination (including arranging for on-site educational programs, monitoring services used, and evaluations program effectiveness), and documentation (including documentation of all services provided, monthly reports, surveys, and annual plan).

QUALIFICATIONS:

B.A. in Social Work, Gerontology, Psychology, or related field with at least two years experience working with older persons or persons with disabilities. Additional work experience may be substituted for the required education. The ideal applicant will have

Knowledge of community support resources, the eligibility criteria for accessing those services and the delivery method; life cycle issues of older persons and families, as well as physical or mental impairments.

Ability to advocate, organize and problem solve with elders and persons with disabilities; establish trust, listen and assist residents in defining their problems; seek input from residents; identify possible solutions and resources; help residents to decide the best course of action; research possible service vendors; negotiate special arrangements and costs; and evaluate services delivered and make adjustments, if needed; counsel or advocate in non-judgmental ways to empower residents to make personal decisions and increase self-esteem and independence; maintain client confidentiality

Basic competency in PC-based software, such as word processing and spreadsheet applications; communication skills in working with people of diverse socio-economic backgrounds, ages, and abilities; mediation skills and the facilitation of group discussions

This position is 3/4-Time, 35 hours/week, Monday through Friday between the hours of 8 and 4:30pm. Beginning wage is $25/hour. Background check required.
