Job Description

The Case Manager is responsible for performing patient care activities as directed by the Clinical Director. The Case Manager will orient and assess new admissions, as needed. The Case Manager’s responsibility is to provide direct support and resources to the client and clinical staff. Support will be provided in the areas of client orientation/assessment, utilization review, discharge planning, care coordination, and other areas as directed.

Responsibilities

· Actively participates in Multidisciplinary Treatment Team meetings.

· Acts as a patient advocate to ensure patient needs are being met and individualization of patient care is prioritized.

· Supports in collaboration with patient’s schools, as necessary and requested by patient’s guardians.

· Completes required assessments and screenings at the directive of the Clinical Director.

· Serves as the liaison between the clinical and utilization review teams.

· Coordinates discharge planning and appointments.

· Ensure discharge appointments are confirmed with patient and loved ones prior to discharge.

· Communicates with referral sources and outpatient providers for continuity of care upon admission, midpoint, and discharge, or more frequently as requested by the provider.

· Facilitates psychoeducational groups at the directive of the Clinical Director.

· Shares pertinent information with team as obtained from client’s family, or others as indicated throughout the program.

· Communicates with the patient’s loved ones and support system, as needed, in regards to patient care, discharge planning, and care coordination.

· Supports in arranging further client assessments or interventions – externally or internally – as indicated to ensure high quality and individualized patient care.

· Facilitates group sessions as needed.

· Assists in gathering pertinent information related to the utilization review process and prepares utilization related documentation as needed.

· Provides holiday and on call/weekend coverage as needed.

· Recognizes, responds to, and assists in psychiatric crisis situations and provide the intervention necessary.

· Responds and assists in medical emergency situations as directed and needed.

· Communicates effectively with client and interdisciplinary team regarding client needs.

· Assists with chart documentation and compliance.

This is a

full-time position working one of the following schedules:

Sunday- Thursday- 8:00a-4:30p

Tuesday- Saturday- 8:00a-4:30p

Qualifications

· Graduate from an accredited college or university with a minimum of Associates Decree in Psychology, Social Work, Counseling, or health-related field and/or qualified CADC or CADC intern.

· A minimum of 1-year experience in chemical dependency - psychiatric setting or related field.

· CEU certifications and on-going training in health-related field

· Knowledge of the Principles and Practices of the discipline.

· Knowledge of Joint Commission Standards.
