POSITION SUMMARYWe are looking for Licensed Vocational nurses (LVN) to fill a number of positions, including on part time basis to join our after hours team to make crisis visits, as well as per diem positions to provide continuous care to our patients.Under the direction of a Registered Nurse, the hospice LVN provides skilled nursing and palliative care to hospice patients. The LVN works cooperatively with other Interdisciplinary Team members and provides intermittent visits or shifts of skilled nursing care, including pain management and symptom control, nursing observations, The LVN also provides information about the expected course of the illness, care techniques and other health measures to the patients, family members or primary caregivers. The LVN participates in the care plan development and implementation. Must be able to utilize computers and be comfortable with EMR.QUALIFICATIONS:Graduate of an accredited school of vocational nursing.Current LVN license within the State. Certification in Hospice and Palliative care encouraged.At least one year experience as a LVN.Valid driver’s license, and auto insurance, reliable/well maintained transportation.Bilingual in Spanish preferred.
