The incumbent will also work with our provider network and community organizations.Primary duties may include, but are not limited to:
Supports team by providing telephonic or face to face outreach to members to assist with gap closures for providers within the Bay Area counties
Responds to telephone, written, and in-person inquiries and initiates steps to assist regarding issues relating to quality interventions, projects, clinic days, and gap in care closures
Supports and promotes State Sponsored Programs through participation in community outreach and events and quality interventions and projects, such as clinic days
Represents State Sponsored Programs in community collaborations
Supports member access to care through home visits, processing of reports, and distribution of collateral materials.
Performs new member orientations and assist with health education
Provides superior, professional, and courteous service to our members, providers, and community organizations.
Qualifications
Requires HS Diploma; 1 year of customer service experience, understanding of the basic principles of managed care and the concepts of publicly financed health insurance such as Medicaid; or any combination of education and experience that would provide an equivalent background.
Comprehends the various cultural and linguistic needs of the Medicaid population, knowledge of the various health and social services available in the assigned region with a special emphasis on services offered by community based organizations, ability to work professionally with the company's associates, community-based organizations, providers, and plan members.
Proficient with Microsoft Office suite preferred, particularly with Access and/or Excel.
Ability to stay motivated and on task in a consistently changing healthcare environment
Ability to work and connect within diverse communities
