Position Overview:The Outreach Coordinator acts as the primary liaison between referring and network providers across Northern California and the assigned Sutter Bay Area service line care givers. The Outreach Coordinator functions as a leader, clinician, and educator, by identifying and developing relationships with potential referral sources including physician specialists, such as cardiologists, cardiac surgeons, neurologists, oncologists and identified primary care physicians to implement programs that foster the professional relationships between the potential referral sources and the Service Line's programs. The Outreach Coordinator provides clinical expertise both to physicians and healthcare staff at referral sources and educates them about specialty programs.This position is an advocate for the assigned clinical service line cardiology, neurology, oncology, and identified Primary Care Physicians to coordinate and expand services, including non-Sutter referring facilities, and facilities in the greater tertiary referral areas. Provides service line support to the area Executives, Medical Directors, and program coordinators in the development, management, coordination, and growth of the regional service line.The Outreach Coordinator demonstrates familiarity with third party payer requirements, EPIC medical record communication tools, and appropriate patient clinical criteria for the successful referral and transfer of patients from potential referral sources to Service Line acute and ambulatory services.This position reports to the Director of Foundation Growth & Medical Group Strategy, in the office of Strategy and Business Development.
