We consistently outperform our peers in every CMS 5-Star Quality Measure, out-score our peers in every CMS 5-Star rating, and consistently achieve a 30-Day Readmissions rate lower than our peers.We are Family Serving FamiliesAs part of the SAN DIEGO HOME HEALTH CARE team, you will have clinical nursing knowledge, physical assessments, teaching and procedure skills, and ability to be directly responsible for assessing, planning providing and documenting nursing care for designated patient in accordance with the physician’s plan of care and agency policies and procedures.The TERRITORY for this PER DIEM (ON-CALL) position is primarily SOUTH COUNTY SAN DIEGO (La Jolla South to the border and East to Santee).Per Diem employees must commit to a minimum of 5 DAYS of availability in a 2 week pay period and must be willing to work some WEEKEND days. .
Graduate from an accredited nursing program.
Must have current, valid and unrestricted state license as a Registered Nurse and CPR certified.
Must have at least one year clinical practical nursing experience in SNF, acute/subacute care.
Home Health experience preferred.
Bilingual-Spanish is a PLUS!
