Hospice of Humboldt is accepting applications for a 3/4-Time Palliative Care Case Manager Nurse.
This position is benefited and approximately 32 hours a week with some flexibility.
$2,500 Sign-on bonus offered
The Palliative Care Case Manager RN is part of a multidisciplinary team providing specialized medical care for people with chronic and/or complex conditions. While focused on community-based palliative care, this role will be cross-trained in hospice care to ensure a seamless transition for patients who may need end-of-life services.
Minimum qualifications are as follows:
1. Graduate of an accredited nursing program.
2. Current California Registered Nurse license.
3. Minimum of one (1) year of experience as a professional nurse within the previous three (3) years or equivalent
experience in a BSN program.
4. Valid driver’s license, proof of acceptable automobile insurance coverage, and reliable transportation.
5. Reliable mobile phone with plan that provides good local coverage.
6. Basic computer skills.
