N Your Home Health Care is a thriving home health agency who has been in business for many years specializing in both skilled care and non-medical care. We are seeking a qualified RN who is, caring, motivated and committed to working along side his/her team of care professionals in a home health setting .
We are currently offering positions available for qualified RN's for the following:
S.O.C.- Admitting patients to agency
Discharging patients
Resumption of patient care
Maintains an ongoing responsibility for assigned caseload.
Regularly conducts Case Care Conferences
Counsels the patient and family in meeting nursing and related needs.
Qualifications:
Minimum 1-year experience
Bilingual a plus
Current CPR certification
Copy of updated Physical, based on state requirements
Copy of updated TB results/ Chest x-ray
Reliable transportation
