Reliant Home Healthcare is seeking a competent Quality Assurance(QA)/Intake Coordinator to work at the Montclair office. Must have a active LVN License, Home Health experience a must.
Candidate must have good communication skills, ability to communicate with Patients, Referral Source and Nurses. Review plan care and clinical documentations
Must be familiar with Synergy Software
Audit of Client Chart
Daily QA of clinical notes
Assist in completing 485/Plan of treatment/Care plan
RN for assessment OASIS, SOC, RECERT AND DISCHARGE
Orientation of Field Nurses
Quarterly QA process
