The LVN performs duties consistent with the Standards of LVN Competent Performance and within the scope of the Nurse Practice Act.KEY JOB FUNCTION:As part of the Elevate Home Health team, you will….

Make home visits, performs preventative treatment, and rehabilitative nursing procedures while implementing the plan of care in collaboration with patients/CG’s, physician, case manager and/or Supervisor as assigned, and documents appropriately.

Assesse need for additional services/equipment on an ongoing basis, as well as home environment to make recommendations that will enhance patient care, promote safety, and prevent adverse effects.

Coordinate plan for patient care outcomes through case conferences with supervising RN and other team members to provide appropriate, effective, and efficient patient care, discharge planning, and documents appropriately.

Work with patient, family, and health care team members effectively to coordinate services,and promote quality of care and positive patient care outcomes.

Update supervising RN/Nursing Supervisor on progress toward goals for plan of care, and implements changes/additions to the POT in collaboration with patient/CG as needs change and goals are met.

Graduate from an accredited nursing program.

Must have current state license as a Licensed Vocational / Practical Nurse and CPR certified.

Must have at least one year clinical practical nursing experience.

Home Health experience preferred.
