Case Manager wanted for home health care company. Duties include but are not limited to:
- coordination of field staff (nurses, home health aides, P.T., etc.) to ensure patients receive the care they need,
- communication with patients and families about healthcare services,
- keeping patient records and completing all needed documentation.
- recommending community services.
Office work Monday to Friday. Some experience preferred, and/or healthcare degree or background, but willing to train less experienced candidates if a good fit. Spanish speaking a plus but not required. Please email cover letter, resume and 2-3 references.
