The TransLatin@ Coalition is seeking a dedicated Outreach & Case Manager for our TRUE program to provide vital support, advocacy, and direct services to Trans, Gender Non-Conforming, and Intersex (TGI) individuals in Los Angeles County. This full-time role combines community outreach with case management to connect individuals with essential resources such as housing, healthcare, workforce development, and legal aid.
Key Responsibilities:
Conduct outreach and engagement to connect TGI individuals with supportive services.
Provide case management, including intake assessments, referrals, and direct assistance.
Assist clients with housing, food security, healthcare, transportation, and workforce opportunities.
Collaborate with community partners to expand access to vital services.
Maintain accurate client records and generate reports to track program impact.
Represent TLC at community events, conferences, and outreach initiatives.
Ideal Candidate:
Passionate about advocacy and community support for the TGI population.
Experience in outreach, case management, or social services (2+ years preferred).
Strong organizational, communication, and problem-solving skills.
Bilingual (Spanish/English) preferred.
Reliable transportation is required.
