Safe Passage Family Resource Center, Fort Bragg, CA seeks bilingual (Spanish/English) Case Manager for the Differential Response program.
-Provide case management to families at risk of having an open FCS court case and have the ability to assess families barriers to create an individualized service plan
-Attend trainings and meetings in Ukiah and other locations in the county
-Work closely with government and other non profit agencies
-Provide support and advocacy to families by attending appointments/meetings with them.
-Transport clients if needed.
-Documentation of all services by maintaining client files and providing monthly and annual service reports
Candidates should have case management experience and who thrives in a client-centered, diverse environment. Prefer at least 2 years experience; 32 hrs/wk. Email cover letter and resume.
