Position Overview:Sutter Gould Medical Foundation (SGMF)is a non-profit, multi-specialty health care organization affiliated with Sutter Health. Based in Modesto, SGMF provides quality medical care, health education and research activities in a five-county area. SGMF's medical staff includes family practitioners, internists and pediatricians, as well as a range of specialist physicians. Services include imaging, nuclear medicine, a health library, health education programs, occupational medicine, diabetes education, nutrition counseling, infertility counseling, and weight managementThe Population Health Nurse has the responsibility to provide care coordination across the continuum of care through telephonic population and disease management. The Population Health Nurse will identify, plan, coordinate, implement, monitor and evaluate appropriate cost-effective health care andutilization of services for patients of the Sutter Gould Medical Foundation in order to optimize patient function and well being. This position provides for optimal outcomes through collaboration with the client, the family, the physician, health plan and other members of the healthcare team.Qualifications:Minimum Requirements

Associate's degree in Nursing

Preferred Qualifications

Bachelor's in Nursing (BSN) preferred

National certification in Case Management (RNC Case Management - RNC-CM)

Public Health Nurse (PHN)

Experience working with chronic medical conditions such as diabetes, COPD, CVA, CHF, ESRD, etc

Experience developing care plans.

Experience working as a member of a multi-disciplinary team.

One "1" year experience in case management, knowledge of health-related information, education, resource and referral services or 5 years general clinical experience

Experience documenting in EMR

Skills and Knowledge

Must have a working knowledge of current treatments and their physical and psychosocial sequelae.

Knowledge of individual and family development over the life span Knowledge of the influence of cultural and spiritual values on health care.

Basic knowledge of applicable laws and regulations, and accreditation guidelines, e.g. CMS (HCFA) IMQ, EMTALA is required.

General knowledge of available health care and community resources appropriate for populations served.

Knowledge of geriatrics including common health & functional changes in the aged, and various chronic disease processes.

Working knowledge of Lean methodology preferred

Must have the abilities to work independently with minimum of direction, to anticipate and organize work flow, to prioritize and follow through on responsibilities.

Must have excellent verbal and written communication, interpersonal, problem-solving, conflict resolution, presentation, time management, positive personal influence and negotiation skills

Ability to work respectfully and creatively with clients of diverse functional abilities, social, economic, and cultural backgrounds to support both client autonomy and client safety.

Demonstrated skills in motivating, mentoring, coaching and redirecting clients in order to achieve compliance with various necessary medical, social and functional interventions.

Leadership ability to delegate, provide direction/guidance to staff and hold others accountable.

Must have strong clinical assessment and critical thinking skills necessary to develop a comprehensive plan of care appropriate to patients with complex medical, emotional and social needs.

Strong attention to detail and accuracy is required.

Ability to work in a high volume case load environment and deal effectively with rapidly changing priorities.

The demonstrated ability to work constructively with a broad spectrum of health care professionals.

Basic computer skills are required.

Must be effective both as a team member and leader.
