We are looking for a Occupational Therapist for the Home Care department at St Joseph Health!Location: Napa, CASchedule: Full time, 80 hours bi-weeklyShift: 8 hour, DaySummary:Provides assessment and treatment of patients requiring occupational therapy services within the Home Care setting. Provides professional occupational therapy visits in the home care setting and directs home health services for caseload of patients and their families in accordance with physician’s orders through direct care, supervision and multidisciplinary team coordination. Is a resource to the Field Staff in the areas of community rehabilitation and individualized rehabilitation needs of the patient. Participates in the utilization review, performance improvement and interdisciplinary conference of the organization. This position reports to the Clinical Manager or Registered Nurses designee.Responsibilities:

Performs the initial and ongoing assessment of patient’s occupational therapy needs and appropriateness for home care; creates and maintains an individualized goal/outcome based care plan based on the physician’s plan of care.

Develops and implements plans of care appropriate to the assessment findings and plans for discharge based on patient’s conditions.

Provides occupational therapy services to the patient in the home setting based on the plan of care, authorizations and physician orders following ordered frequencies and duration within the Medicare week.

Adheres to agency policies and procedures for nursing, infection control, bio-hazardous waste management, quality improvement activities, safety and risk management practices.

Confers with the physician in developing and revising the plan of care; reporting patient progress status; obtaining and documenting additional orders based on the initial and ongoing assessment of the patient progress; following regulatory guidelines and policies in writing orders.

Evaluates the patient for adaptive equipment needs and obtains physician orders; obtains needed adaptive equipment.

Instructs the patient regarding active and passive muscle; activities of daily living; use of assistive devices; and safety.

Coordinates care activities among all disciplines on the care by participating actively in case conferences, identifying additional services needed and discharge planning with multidisciplinary team and other resources to achieve patient outcomes.

Makes ongoing therapy assessments, evaluates patient progress, updates the plan of care based on findings, and reports significant changes in the patient’s condition to the physician, appropriate team members and/or Clinical Manager.

Participates in care coordination by communicating with other disciplines on the case regarding patient progress, changes in the plan of care, and discharge planning through written documentation and participation in case conferences.

Accurately and thoroughly documents patient care observations, interventions, and evaluations; prepares and submits completed documentation of the home care visit within the established time parameters in agency policy.

Maintains patient schedules based on orders and authorizations within the Medicare week; submits caregiver schedules timely and accurately; communicates scheduling changes daily; meets productivity standards demonstrating effective time management skills.

Maintains daily communication with Scheduling Coordinator providing update to visit activities and changes.

Participates in utilization review and quality improvement activities of the organization.

Demonstrates a high degree of commitment to customer service and provision of quality care.

Additional Responsibilities:

Serves as resource to Management and Field Staff of the organization.

Supervises the certified Occupational Therapy Assistant when appropriate to the provision of patient care.

Participates in utilization review and quality improvement activities of the organization.

Reviews and recommends occupational therapy policies and procedures for the organization that are in compliance with Medicare, state, federal and credentialing criteria and standards.

Completes in-service on blood borne pathogens annually, age-specific training; annual clinical competencies and other required competencies.

Participates in Corporate Compliance Program.

Must participate in age-specific training annually.

Participates in staff meetings, performance improvement activities and clinical record review.
