Job Summary:Reporting directly to the Community Health Operations VP and the Senior Vice President of National Medicaid, this position will be responsible for the execution of our national strategy to address state and safety net priorities for Medicaid enrollees outside of KP. As part of the portfolio, the position will be responsible for partnering in innovative ways with safety net providers, including Community Health Centers (CHCs) and other safety net providers. KP’s approaches include direct grantmaking through Community Health programs, care delivery partnerships with CHCs, and innovative projects that leverage the mutual strengths of KP and our safety net partners to drive quality and population health improvement. This position will be responsible for grantmaking strategy pillars which include efforts focused on virtual care, quality improvement, EHR adoption and optimization, social health, specialty access and core support in the safety net sector. The leader will focus on these pillars to be responsive to safety net needs and where KP can leverage its unique assets to have the most impact. In addition, the position will be responsible for KP’s Medi-Cal PHM Initiative. KP committed to support the Medi-Cal program and Medicaid goals by partnering with community health centers (CHCs) statewide to advance population health management (PHM) capabilities. The Medi-Cal PHM initiative, in partnership with the CA Department of Health Care Services (DHCS), and the California Primary Care Association (CPCA), will focus on advancing the community health center PHM capabilities to achieve improved population outcomes. Key components of the work are: Care models to enable whole-person care through redesigned care teams and evidence - based practice to improve PHM capabilities at CHCs

A PHM platform that enables delivery model, includes bi-directional data exchange, provides a whole patient view, and facilitates coordination with community resources

Implementation support for CHCs to enable PHM care delivery model through curriculum, collaborative learning events, practice coaches & tools/resources

Specialty access models for Medicaid beneficiaries (non KP members) being served by CHC

An ability to lead people across a matrix through influence is an essential capability for this leader. A strong track record of working with or collaborating with state agencies and safety net delivery systems in California is also required. This leader will need to effectively partner with KP leaders to align agendas and initiatives in support of safety net. S/he will also need to excel at managing relationships with multiple stakeholders both within the organization as well as external leaders across the state to execute on these strategies and achieve impact for population health outcomes for Medicaid enrollees in California. This leader will also need to have strong vendor management skills to supervise specialized consulting contracts, critical to the success of the population health management initiative. This position is responsible for managing the cohesive quality and timely performance of our PHM efforts, while fostering a workplace environment consistent with the values and mission of Kaiser Permanente.

Essential Responsibilities:Serves as the accountable leader for the CA safety net PHM strategy including: oversight and management of a complex budget as well as staff and multiple contractors working on the portfolio of interventions; meeting initiative goals and timelines; and managing relationships within KP and external to the organization to ensure success

Oversees relationships with the CA state agencies (CA Health and Human Service, Department of Health Care Services) and safety net leaders on implementation of the population health strategy

Coordinates efforts internally with medical groups and health plan leaders to leverage necessary resources to develop population health care models and platforms for the safety net (i.e., stratification, care coordination and complex care management, patient engagement, social and community supports)

Ensures our PHM strategy, programs, and initiatives are operationalized through effectively designed team roles, structures, practices, norms, culture, systems, budgets, shared services, vendor relationships, program management, and evaluation, driving visibility and accountability to success metrics and evaluation – ensuring maximum effectiveness and productivity of the PHM portfolio

Coordinates with the SVP for Delivery System Strategy and Innovation to export the population health solution for the safety net into a market competitive PHM solution that can be leveraged as a product or service outside of CA.

Selects and manages multiple specialized consulting contractors that are critical to deliver our PHM and specialty access strategy

Serves as an internal expert for federal and state health policy strategy and advocacy in matters relating to population health, social health, and care coordination for vulnerable populations

Collaborates with Community Health leaders to align social health and safety net initiatives in support of the CA population health strategy

Leads and influences stakeholders across KP to build awareness of safety needs and marshal resources to achieve strategic goals related to supporting the safety net

Provides input to KP’s national and state level Medicaid strategies with respect to the types of populations and related care models that KP should strategically focus on for growth

Collaborates with the Chief Medical Officer for Health Plan Quality and SVP for Community and Social Health on population health solutions to advance the enterprise equity agenda related to vulnerable populations

Basic Qualifications:

Experience

Minimum ten (10) years of experience in Medicaid care coordination and/or delivery leadership in health systems, physician groups and/or health plans.

Education

Bachelor-s degree required.

Additional Requirements:

Deep experience working with California safety net providers

Expert in care delivery strategies that deliver excellent outcomes on tight budgets for Medicaid and vulnerable patient populations

Deep knowledge of Medicaid managed care programs across several states

Particular knowledge and experience with Medi-Cal managed care including Medi-Cal reforms related to CalAIM

Demonstrated successful leadership skills in executing large programs

Demonstrated experience in leading a highly matrixed organization through influence rather than organizational structure

Experience leading complex organizations and teams to execute on strategies to meet short- and long-term growth and operating goals

Preferred Qualifications:

MD, MBA, MHA, MPH or other equivalent degree or experience preferred.
