The Bilingual Care Coodinator provides strength-based, culturally competent Wraparound services, including Wraparound Family Team facilitation, group leadership, and coordination of those providing services to Wraparound children and families, including the Parent Partner and Youth Partner Staff.

Location:

Anaheim, CA

Responsibilities:

Maintain regular contact with the Parent Partners and Youth Partners.

Guide the Family Team through the four phases of Wraparound.

Maintain a caseload of eight to ten families, or as otherwise determined by the County.

Respond to family emergencies according to the Family Plan including connecting to services available through the Provider Network.

Assemble the youths Family Team within three weeks of case assignment by interviewing the youths family and identifying family members, natural supports, agency representatives, and other significant persons.

Coordinate the meeting with the youths Family Team and develop the individualized Plan of Care based on the Family Teams strengths and needs. The Plan of Care shall include a comprehensive 24-hour safety plan. The Plan of Care shall reflect the best possible fit with the culture, values, and beliefs of the client and family.

Assist the youth and the youths Family Team to access needed resources and strength-based mental health services, social services, education services, and other supports and services

Provide on-call crisis/emergency services as needed for youth and families who access the programs 24 hour crisis phone line.

Discuss the provision and quality of activities actually provided with the youth and youths Family Team and ensure that the activities being provided are responsive to goals and needs identified in the Plan of Care.

Provide transportation for the youth to appointments or to access services as required.

Maintain all required documentation in a timely manner.

Qualifications:

Bachelors Degree in human services, psychology, child and adolescent development, social work, or related field from an accredited college or university required.

Must be at least 21 years of age.

Bilingual Spanish/English required

Experience working with children who are part of the child welfare system; experience working with children under age five a plus

Ability and willingness to work independently in the office and in the community.

Valid California drivers license and ability to be insured by Senecas insurance carrier

Own vehicle, valid California Driver's License and insurance.

Schedule:

Monday - Friday

Flexible schedule required, with the ability to work some evening hours
