GENERAL DUTIES:Theindividual in this position performs psychosocial assessments and develops aplan to meet the needs of patients with complex social issue/problems, andarranges post-discharge services for patients as needed. This individual’s responsibility willinclude, but not limited to the following activities: a) assist with adoptions,abuse and neglect cases, including making referrals to state and federalagencies b) participate in the weekly Complex Case Review meeting and the dailypatient care conferences c) provide resources and make referrals to communityagencies for ongoing counseling and or continued care d) maintain an updatedlist of providers for post-discharge care e) collaborative communication withpost-discharge providers f) update Case Managers on variance to post-dischargeplans g) monitor and provide reports to the DCM and hospital leadership as requestedh) maintain knowledge of regulatory requirements and community resources asassigned. MSW will seek out supervisionas needed with a Licensed Clinical Social Worker. Will use Patient Medical Record, HPF, andAllscripts as primaryinformationsystems.AttendsHospital orientation, workshop led by Director of Case Management or designeethat cover the Tenet Case Management Program, focuses on Discharge Planningarrangements and other related topics.DEPARTMENT/CLINIC SPECIFIC DUTIES:Priority 1: Assessment, development and revision of planfor patients with complex social issues/problems and issues specific tospiritual and cultural values with consideration of patient developmentalstages (60% daily, essential).Priority 2: Assist with adoption/abuse/neglect cases andreporting of appropriate cases to state and federal agencies (5% daily,essential).Priority 3: Participate in Complex Case Review (weekly)and daily patient care conferences (5% daily, essential).Priority 4: Coordinate/arrange post-discharge services(15% daily, essential).Priority 5: Ensure Case Managers and providers are keptup to date on variances to post-discharge plans for patients (10% daily,essential).Priority 6: Maintain knowledge of internal policies andState/Federal regulatory requirements related to post-discharge planning andsocial services (5%, essential).Priority 7: Coordinates daily work activity of socialwork staff.Priority 8: Assist staff in assessing patient needs andsage discharge plans.Priority 9: Complete weekly schedules, monthly on-callschedules and holiday coverage.Priority 10: Completeand maintain Social Services payroll.Priority 11: Completeyearly evaluations.Priority 12: Will actas preceptor to staff and student interns.Priority 13: AssistsDirector of Case Management to staff qualified employees by providingsignificant input into the hiring of personnel as well as orientation andtraining to new staff.Priority 14: Assists Director of Case Management withSocial Services Performance Improvement Plan.Priority 15: Ensurecompliance and requirements of all regulatory agencies.POSITIONQUALIFICATIONS:

Mastersdegree in social work from an accredited University.

Current,valid licensed clinical social worker (L.C.S.W.) in the State of California.

Threeyears social work experience in an acute hospital setting, includingadministrative and supervisory duties

Excellentverbal, written and computer skills.

Knowledgeof internal policies and State/Federal regulations related to dischargeplanning and social services.

5150certification (must have within 6 months of hire date.)
