Offering treatment for malignant diseases, including lymphoma, myeloma, leukemia, myelodysplastic syndromes, and selected solid tumors, our program strives to integrate each patient into normal, everyday life.BMT Multidisciplinary Care Coordinator is a Registered Nurse specializing in the care coordination for the BMT Pre/Post Transplant/Car T patient population. - 10 Hour DaysAs a BMT MCC (RN), this role will provide assistance in planning, coordinating, and delivering patient care for selected patient populations across the continuum of care using the nursing process, ensuring compliance with physician orders and established policies and procedures. The BMT MCC, is a Registered Nurse (RN) who provides coordination and care of Stem Cell Transplant patients requiring assistance and support in accessing and managing their health care needs. They collaborate with other health care providers to ensure continuity of care and coordination of a wide variety of patient services. They will use performance improvement methodology to design data collection systems / processes and identify opportunities to improve care / service for patients. Responsibilities include coordinating patient contact, patient education, and preparing patients/families for procedures/tests in collaboration with the health care team; collaborating with others to ensure continuity of care and coordination of a wide variety of patient services; identifying opportunities to improve patient care services; assisting clinic management to identify and develop operational needs for the clinic and to improve patient care services; and they may provide direct patient care.Equal Opportunity Employer Stanford Health Care (SHC) strongly values diversity and is committed to equal opportunity and non-discrimination in all of its policies and practices, including the area of employment. Bachelor's degree or equivalent from an accredited college/university

Valid California Registered Nurse (RN) license

Basic Life Support (BLS) Certification for Providers from the American Heart Association

2 years of experience as a registered nurse (RN) minimum

1+ year in ambulatory care or community health nursing (preferred)

Experience with Hematology Oncology

Bone Marrow Transplant (BMT) experience preferred
