Please contact Human Resources at this medical center and speak with the Education Debt Reduction Program Coordinator for additional information.ResponsibilitiesThe VA Palo Alto Health Care System Pharmacy Service provides pharmaceutical care services to our 300-bed acute care hospital division (Intensive Care Units, OR/Recovery Room, Medicine, Surgery, Sub-Acute Medicine/Long Term Care, Hospice, Spinal Cord Injury, Polytrauma, Psychiatry, Emergency Department) with centralized operation for Unit Dose, Ward Stocking and Sterile Compounding medication preparation and dispensing. We have a fully integrated computer system including online provider order entry, documentation of medication administration using barcode technology, and automation of the unit dose distribution system. The Incumbent serves as the program manager of all sterile and non-sterile compounding processes, as well as hazardous drug procedures for VAPAHCS. Responsibilities shall include but are not limited to: ensuring compliance with all federal and regulatory standards at all stations involved in USP 795, 797, 800 and/or 825 activities; ensuring timely training of pharmacists, technicians, trainees, and other supportive personnel involved in compounding; serving as a resource and liaison between pharmacy and other services. This Program Manager is required to have extensive knowledge of daily operations involving the Clean Room and compounding activities as assigned. The position will require some travel to other VAPAHCS sites; as some USP sections are applicable at our locations outside of Palo Alto. There may also be travel to other VA sites within our VISN. General duties assigned to this position include: Program Management Activities:Creates and/or modifies internal Pharmacy Policy and Procedure manuals and HCSM (health care system memoranda) relevant to the USP requirements; develops and maintains staff competencies and provides training for all USP requirements for Pharmacy staff and assists EMS, Nursing, and other services with developing/modifying their staff competencies; manages environmental certification testing, results, and corrective actions; maintains and updates all USP documentation; performs routine quality assurance audits of USP compliance; collaborates with OFPD, Engineering, EMS, Safety, Nursing, Occupational Health, Supply Chain, and other affected services to ensure USP compliance; represents Pharmacy on various healthcare system wide, VISN and National committees and projects (e.g. IP&C, CSP Advisory Council, VISN 800, etc.) as assigned in various roles (co-chair, member, etc.); presents USP updates and compliance to healthcare system leaders monthly and quarterly; assists with OIG, VORP, FDA, EPA, and TJC surveys as assigned; assists procurement with inventory management as assigned. Sterile Compounding Activities:Will occasionally be scheduled as the daily IV Room Pharmacist position, to cover days off and vacation of routinely scheduled IV Room Pharmacist. This not only provides coverage for smooth and timely IV Room workflow, it also allows the Program Manager to observe first hand processes and workflow in the area to identify opportunities for re-training, conduct compliance checks, and potential changes to workflow. As the covering IV Room Pharmacist, duties include medication order screening, verification, assisting pharmacy technicians with preparation, checking all IVs (piggy backs, large volume, syringes, OnQ pumps, chemotherapy, TPNs, etc.) prepared before dispensing to patient care areas, and assurance of completion of all daily environmental cleanings. Will be cross-trained as a potential back-up to the Ambulatory Infusion Clinic Pharmacist. Serves as a preceptor to trainees, students, and residents as assigned. Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Preferred Experience: Extensive knowledge of applicable USP chapters (795, 797, 800, 825). Extensive knowledge of operations within a sterile IV clean room. Completion of a PGY-1 Pharmacy Residency. Grade Determinations: GS-13 (a) Experience. In addition to the basic requirements, must have 1 year of experience equivalent to the next lower grade level. (b) Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. 2. Facility Program Manager. Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomic or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.b. Expert understanding of regulatory and quality standards for their program area.c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
