SummaryThe Patient Safety Consultant assists the Patient Safety Manager in assuring and maintaining the Patient Safety Program under the Office of Quality, Safety, and Value (QSV). The Patient Safety Consultant is a member of the QSV team and is involved in assessing, delivering, and improving the quality, safety and value of healthcare services at the VA Palo Alto Health Care System (VAPAHCS).ResponsibilitiesTHIS IS AN OPEN CONTINUOUS ANNOUNCEMENT WITH A CLOSING DATE OF SEPTEMBER 30, 2020.The initial cut-off date is 02/28/2020Applications will be referred every 2 weeks.Applications will remain on file for 3 months. The Patient Safety Consultant demonstrates effective administration of functions that include, but is not limited to clinical screening, data compilation, documentation and entry into a data base, comprehensive identification and reporting of all quality monitoring, establishment of work groups to accomplish tasks, VISN and National collaboration toward common goals, and the maintenance of confidentiality that meets and/or exceeds all standards outlined in regulation and law.The major duties and responsibilities include, but are not limited to : Meets the Nurse Qualifications Standard four (4) dimension requirements: Practice (Practice, Ethics, Resource Utilization), Professional Development (Education/ Career Development, Performance), Collaboration (Collaboration, Collegiality), Scientific Inquiry (Quality of Care, Research), and Customer Service in his/her grade level. Attends VISN/National work groups and conferences pertaining to her/his duties as directed by the Director of Quality & Safety. Works with Patient Safety Manager, uses the Safety Assessment Code (SAC) Matrix to determine if action is required for a particular event or near miss (i.e. Root Cause Analysis (RCA), Administrative Investigation, Peer Review Area, etc.). Assists in medical center activities and preparations for The Joint Commission (TJC), Office of the Inspector General (OIG), and other accreditation and external reviews.RequirementsConditions of EmploymentCONDITIONS OF EMPLOYMENT: You must be a U.S. Citizen to apply for this job. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency, and accredited by the National League of Nursing Accreditation Commission (NLNAC) which changed to the Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE) at the time the program was completed; OR the completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005/27, Appendix G6). OR In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration (i.e., licensure) as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or the District of Columbia. Note: Provide your RN registered license number on your application. Grade Determinations: In addition to the basic requirements, the following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience. Independent leadership role (formal or informal) in a complex hospital setting. Experience utilizing systems redesign principles to improve healthcare systems.
