automotive care.Position SummaryReporting to the Vice President of Quality, Education and Compliance,
this position is responsible for coordinating and/or presenting the
orientation of new staff as well as coordinating and/or presenting
ongoing education for staff. Coordinating student experiences with the
students, schools and instructors to facilitate community health field
clinical placements. Tracking records of completion of clinical
competencies. Conducting Quality Assessment and Performance Improvement
(QAPI) activities as indicated by the annual QAPI Program plan and at
the direction of the Vice President of Quality, Education and
Compliance. Participating on the QAPI Committee. Conducting chart
audits, including comprehensive chart audits, utilization management
audits, infection control audits and benchmarking report audits.
Educating staff one-on-one when identified documentation or education
needs have been identified. Assisting with preparation for and support
during accreditation deemed status survey(s) and other surveys, such as
Kaiser and Department of Public Health (DPH) complaint surveys.Essential FunctionsClinical Staff Orientation and Competencies ? Coordinating with managers and human resources re: start date
for new employees and travelers? Assuring a basic schedule of classroom orientation activities
for a new employee's orientation period is completed by the manager, and
that field orientation dates have been calendared for first three weeks? Explaining to employee all elements of orientation program and
outlining employee's responsibility in completing orientation program
and necessary materials? Meeting with new patient care employees during classroom
orientation to assure that the employee's learning/orientation needs are
met, and? assuring new employee skills self-assessment is completed and
routed to mentor in order to individualize new employee clinical
orientation? Providing education on aspects of new employee orientation? Meeting with new patient care employees during classroom
orientation to assure that the employee's learning/orientation needs are
met? Coordinating implementation and follow-up of competency
requirements and testing of patient care staff with appropriate Clinical
Team Manager(s)? Maintaining a record of competency completion, communicating
with supervisors if an employee is not competent or needs further
orientation/supervision? Communicating with employee's supervisor re: progress in the
orientation process? Revising orientation program, as needed, in collaboration with
the Vice President of Quality, Education and Compliance (VP, QEC)? Providing orientation booklet, paperwork, and orientation
session, as appropriate, for students and contractorsClinical Staff Education ? Using information collected at Quality Assessment and
Performance Improvement (QAPI) Committee Meetings, Safety Meetings,
staff surveys, results of and changes in competency program and changes
in regulations, contributes to development of a staff education plan
annually? Preparing and distributing staff education needs survey
annually? Participation in education plan development including
suggested ways for information to be presented? Monitoring changes in requirements by Board of Registered
Nursing and Board of Behavioral Sciences in providing Continuing
Education Contact Hours and maintaining agency's status as a Continuing
Education Provider? Designing and coordinating in-service education programs, as
assigned by the VP, QEC? Arranging with Team Manager to assure that all staff arranged
for by contract have completed competency requirements? Communicating with appropriate Team Manager about continued
learning needs of employees? Acting as Agency resource for student field placements,
assuring contract in place, obtaining class objectives and course
outline from school, working with Clinical Team Managers for assigned
preceptor and schedule, monitoring performance and communicating with
instructor, as assignedIn-Services ? Coordinating and implementing mandatory education classes for
Home Health Aides, which meets the 12 CEU's offered per calendar year
regulation, as assigned? Coordinating with in-service provider needs for equipment,
copies of handouts, etc., as assigned and in collaboration with Quality,
Education and Compliance Coordinator I (QEC I)? Providing appropriate sign-in sheet for in-services in
collaboration with QEC I? Greeting and introducing speakers or arranging for alternate
presenter? If Continuing Education Units are being provided: collecting
speaker's CV or resume and course objectives; arranges for certificates
to be provided to participants who require Continuing Education Units,
as assigned and in collaboration with QEC I? Maintaining binder of all education programs provided for the
year according to specified regulations and in collaboration with QEC IQuality Assessment and Performance Improvement ? Participating as a member of the QAPI Committee? Conducting medical record audits, including comprehensive
chart audits for assigned team, utilization management hospice
eligibility audits , and discipline-specific audits, with documentation
gaps communicated to clinicians? Identifying major deficiencies from chart audit tallies,
updating QAPI Power Point slides, according to quarterly data findings,
and presenting information at QAPI Committee meetings? Participates in Performance Improvement Projects (PIPs), as
assignedSafety and Infection Control? Participating as a member of an Agency safety committee, as
assigned? Calendars and assures provision of Agency CPR classes, in
collaboration with contracted vendor? Acting as designated Agency Infection Control Nurse in
providing resources to staff, including information from CDC and County
Health Department, as assigned? Coordinating infection prevention and control activities and
surveillance in the Home Program(s) and Bruns House, including
monitoring the effectiveness of the IC program? Monitoring patient and staff infection data quarterly for
trendsPosition Requirements? Must have the ability to maintain confidentiality regarding
patients and staff? Sensitivity to the various needs of different levels of staff
and ability to teach the adult learner? Clinical knowledge of hospice and home health practice? Demonstrated ability to interface with an interdisciplinary
team? Knowledge of the quality assessment and performance
improvement (QAPI) process? Detail-oriented, with ability to communicate effectively with
all staff members? Knowledge of the Board of Registered Nursing and Board of
Behavioral Sciences and RCFE and Nursing Home Administrator Program
requirements as a Continuing Education Provider Knowledge of Infection
Control Practices/RegulationsExperience & Education? Current California RN License required? Computer skills required, including Word, Excel and PowerPoint? Bachelor's degree in nursing or other health-related field
preferred? 3-5 years' experience in hospice and/or home health preferred
