This position is a regular, full-time, benefited position. We are open to exploring a flexible schedule or other part time hours while still meeting the needs of the program. This position The Registered Nurse (RN) is a part of our Redwood Coast PACE team and
Provides direct and indirect nursing care to PACE participants, working in close coordination with PACE
primary care providers to manage ongoing care and complex clinical situations in the PACE Center and
Clinic.
Conducts assessments and reassessments and assures Nursing Level of Care documentation is complete.
Is an active participant of the Interdisciplinary Team and coordinates accordingly to develop plans of care
for PACE participants.
Qualified candidates will have graduated from an accredited school of professional nursing and have an active RN license as well as three (3) years of nursing experience with a minimum of one (1) year caring for the elderly or frail.
