Position Description: The Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) assumes responsibility and accountability for the application of the nursing process and the delivery of patient/client care and/or contracted services under the supervision of a registered nurse or licensed designee. The LVN/LPN provides service in accordance with Maxim philosophy, standards and policies. Essential Duties and Responsibilities:Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or
contracted servicesDemonstrates on each assignment the provision of skilled nursing care rendered in accordance
with prescribed ordersIdentifies and documents changes in patient’s both physiologic and psychosocial condition
Notifies Director/Supervisor if applicable on the following;
a. Physician notification to obtain necessary orders for intervention(s) per stateregulationsb. Changes in patient/client conditionAdheres to Maxim's policies and procedures and applicable protocols
Identifies and prioritizes actions based upon patient/client care requirements
Assures patient safety at all times and recipient of services provided
Verifies that documentation reflects patient/client collaboration, participation in plan of care,
expected patient/client outcomes, and selected nursing interventionsPerforms interventions, treatments, and medication administration, according to identified
priorities, plan of care, facility policies and patient/client care outcome standards as applicable.Monitors and documents effectiveness of interventions, treatments, and medicationsEvaluates patient/client and provides a safe, comfortable and therapeutic patient/client
environment
Monitors the safety and effectiveness of the environment and equipment including universal
precautions
Participates in activities to facilitate good interpersonal communications
Communicates observations and significant changes in patient/client status to the Director of
Clinical Services and/or Clinical Supervisor /Case Manager or licensed designee
Documents all physician collaboration/coordination of care in the medical record and
communicates this information as neededAttends case conferences, team meetings, education in-services as required
Supports professional practice by observing the following;
a. Nurse Practice Actb. Standards of Nursing Practicec. Standards of Patient Cared. Standards of Accrediting Body (ACHC, Joint Commission, etc.)e. State and Local Lawsf. Patient Bill of Rightsg. Maxim Healthcare, Nursing, and Client’s Hospital PoliciesPerforms other duties as assigned
Minimum Requirements:
Graduate of an accredited practical nurse or vocational nursing program, preferred
Licensed Practical Nurse or Licensed Vocational Nurse in specific state
Complies with accepted professional standards and practice
At least one year prior professional nursing experience, preferred
Willingness to participate in skills advancement training or novice nurse program if applicable
Current CPR as applicable, for direct patient care
PPD or chest x-ray if applicable
Successful completion of new hire training as applicable to job site
Participation and completion of Maxim’s Competency Program
Strong verbal and written communication skills
Current Health Certificate (per facility contract or state regulation)
Must meet all federal, state, and local requirements
