Job Description

Responsible for patients referred to the Organization; identifies insurance coverage, plans and coordinates referrals for home health services. Responsible for establishing and maintaining positive relationships with customers and referral sources within an assigned territory to promote home health services

1. Attains a completed copy of the patient’s history and physical, visit notes, orders and any documentation pertaining to the need for home health services.2. Identifies and verifies insurance coverage of home health care services.3. Consults with physicians, nurses, social workers, discharge planners and other disciplines to establish a coordinated home plan of care. Send completed patient visit notes as requested.4. Completes and assist referral process for CBP (Call Back Program) admissions.5. Interviews the patient, family, and caregiver and discusses the home situation, current needs, and any psychosocial factors that are relevant to the plan of care.6. Completes referral information that includes intake data, essential background information, hospital course, and the plan of care.7. Maintains liaison relationship with community partners and insurance personnel, providing information and education on organization services, coverage issues and related areas.8. Participates in patient satisfaction surveys and coordinates patient concerns to branch leadership.9. Attends meetings and participates on committees as required.10. Informs Director of Patient Care Services and Intake Supervisor of all delays in clinical care.11. Performs other duties as assigned by the Patient Relations Manager or Vice President of Business Development.

Qualifications

1. High School Diploma or G.E.D. required.2. Self-directed and able to work with minimal supervision.3. Demonstrate excellent observation verbal and written communication skills.4. Exercise professional organization skills.5. Excellent customer service communication.6. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
