Position Overview:POSITION SUMMARY:Under the general direction of the Care Coordination Manager or Director, this position plans, organizes and implements the provision of comprehensive social work services to Medical Center patients and families. It is a Centers of Medicare and Medi-Cal (CMS) mandate that social work services be provided to assist patients and their families/support systems in coping with the psychological, social and economic factors affecting their hospitalization and discharge planning. Licensed Clinical Social Workers provide mandated reporting, crisis intervention, information, referral and discharge planning. Other duties may include supervision of associate social workers, participation in officer of the day, on-call and/or onsite weekend social work coverage in order to provide social services on a 24/7 basis.Qualifications:MINIMUM POSITION REQUIREMENTS:Education:

Current California Licensed Clinical Social Worker.

Master’s Degree from a School of Social Work, accredited by the Council of Social Work Education.

For NICU: California Children Services (CCS) Paneled preferred

Experience:

Minimum of two (2) years in acute medical social work or similar health care setting and clinical supervisory experience preferred.

Experience with clinical assessment for patient with complex medical, emotional and social needs.

Basic computer skills and proficiency with Microsoft Office applications including Word, Excel, and Power Point.

Knowledge:

Ability to practice clinical social work at an advanced practitioner level.

Knowledge of available health care and community resources

Demonstrates knowledge of psychosocial issues and community resources relative to age and disability /illness of diverse patient population.

Demonstrates knowledge of medical terminology.

Ability to evaluate patient and family needs to effectively plan and implement social work interventions and discharge planning within the health care setting.

Communicate effectively orally and through written and electronic documentation.

Knowledgeable in working with an interdisciplinary team in a dynamic setting and maintain flexibility to adjust priorities.

A broad knowledge base of health care delivery and case management within a managed care environment

Practice in accordance with applicable state and federal regulations, statutes, and laws

Adherence to the NASW Code of Ethics and Standards

Special Skills/Equipment:

Excellent verbal and written communication skills.

Demonstrated leadership skills

Able to successfully navigate the electronic health record (EPIC) for documentation of assessment, interventions, involvement of patient/family and discharge planning.

Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components.

Ability to interact effectively with key internal and external constituents using collaboration, and customer service skills that promote excellence in patient experience.
