We are looking to hire an RN who is Bilingual (English/Spanish) speaking.

As an RN, Registered Nurse Hospice you will:

Have flexible hours and working close to home (reduce your commute!)

Apply specific criteria for admission and re-certification to establish patient's eligibility and appropriate levels of care.

Assess the impact of the terminal diagnosis on patient's physical, functional, environmental, safety, ADL and psycho-social needs, including risk for pathological grief.

Assess all aspects of the patient's pain and develop an individualized pain management plan.

Assess the ability of the caregiver to meet the patient's immediate needs at start of care and throughout care.

Develop an individualized palliative plan of care based on the needs identified which demonstrates management of discomfort and provision of symptom relief.

Support, consult, and educate the patient, family and caregiver regarding the disease process, self-care techniques, end-of-life cares and managing ethical concerns.

Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.

Participates in the On-Call rotation and as requested (This could be a day or two per month up approximately 4-5 days a month.

Qualifications:

Current, active RN License in the State of California by the Board of Registered Nursing.

Current CPR, health clearance and clinical skills self-assessment.

Hospice experience

Minimum of one year of professional nursing experience within the last 3 years.

Clinically sound/confident in skills and understanding of disease trajectory- Recent acute care experience is preferred.

Has compassion and an approachable personality but also strong enough to be confident and give directions to family and the IDG team.
