- 10+ year experience in clinical and managerial roles in a multi-faceted health care system and multi-service provider setting required.
- Clinical license (e.g., California RN) or certification in a Continuum discipline (e.g., SNF Administrator or ACHE Certification) preferred.
- Bachelor’s degree in health services, business administration, public health, nursing or related discipline required; master’s degree preferred.
- Must have experience and/or knowledge of Care Coordination or experience in the Continuum of care.
- Clinical experience and license in one of the following: RN, Physical therapy or pharmacy license required.
