ABOUT THE POSITION:

As Care Coordinator for Hospice of Santa Barbara, your primary responsibility will be to provide administrative support to the Social Work Care Management team. You will work in concert with a Care Manager to help facilitate quality care management services to Patient Care Service (PCS) patients and their families. You will perform a vital function on the team by smoothing the flow of paper work and information to and from patients and community agencies. Acting behind the scenes, the Care Coordinator will alleviate pressure on Care Managers by assisting them with home visits, filing, data entry and collection; this will help the Care Managers maintain a high level of efficiency and output. This role is a full-time position (40 hours per week) and includes health insurance, retirement, vacation and sick time and holiday benefits. Specifically, your essential duties and responsibilities will be as follows:

Provide supportive home visits to patients/families; attend medical appointments with patients

Attend daily team meetings, 90-day Plan of Care meetings, HSB staff meetings, and PCS monthly meetings.

Participate in community outreach and events as needed or directed.

Attend weekly supervision with PCS Manager.

Collaborate with PCS team, volunteers, and other health care professionals involved with patient's care.

File paperwork on behalf of assigned clients.

Make contact with collateral sources.

Inform clients of community resources.

Record visits and other important information using web-based client tracking software.

WHAT YOU'LL NEED:

Bilingual skills (fluency [read/write/speak] in English & Spanish)

Demonstrated ability in care management and/or counseling, including a familiarity with community resources

Sensitivity and commitment to the hospice philosophy of care

Knowledge of Santa Barbara medical, health care agencies, and social service agencies

Experience in public speaking; effective presentation skills

Professional demeanor

Excellent customer service and follow through skills
