Harbor Supported Living Services is looking for bright, energetic, committed people who believe strongly in social justice for all.

Harbor SLS is seeking Community Support Staff to assist, teach, and serve individuals with developmental disabilities/intellectual disabilities to be as independent and integrated into the community as possible. To that end, we are seeking passionate staff to support individuals, both in their own homes, and in the community. Job duties are as diverse as the individuals we serve: the job could involve assisting someone at home with cooking, cleaning, and other forms of household management. Alternatively, it could mean assisting individuals in a college class, attending community events, or getting exercise in one of the many beautiful State parks in the Santa Cruz County area.

We are currently looking for people who would be available for weekend, evening, daytime and overnight shifts to assist people with developmental disabilities who live in their own homes. We have both full time and part time positions available, as well as, livein housemate positions. Experience in the field of developmental disabilities and work experience with individuals with challenging behaviors preferred but not mandatory. COMMUNITY SUPPORT STAFF JOB DESCRIPTION

Responsibilities:

Community Support Person's provide Training and Habilitation services and instruction, as well as, Personal Assistance to individuals who live in their own homes, neighborhoods and/or at job sites. Under supervision of the Coordinator, Community Support Person's will provide support to individuals who have one or more of the following challenges or needs: developmental disabilities, physical challenges, traumatic brain injury and/or mental health support.

Agency Philosophy:

Assist and support individuals to choose and live in their own homes, have profitable jobs, develop friendships and participate in community life and activities.

Duties:

Implement training and support services to individuals as specified in individual support plans. The areas of training and monitoring include, but are not limited to: housing, personal finance, mobility and accessibility, health and safety, relationships, recreation, self advocacy, job training, decision making, hygiene, personal care and problem solving.

Access individuals' abilities in the above mentioned areas.

Participate in the development of support plans.

Maintain all necessary individual records and evaluations

Participate in the individual(s) team meetings to discuss services.

Provide input for program planning and implementation.

Communicate as required/requested with your Coordinator concerning services, training, etc., with each individual you work with.

Knowledge, Abilities and Skills:

Knowledge of the needs of persons with developmental disabilities, possess the skills to effectively teach persons with disabilities: demonstrate the ability to encourage person with disabilities to make informed choices and knowledge to facilitate opportunities for growth in the individuals being supported.

Must be able to work with minimal supervision. Must have good communication skills.

Training and Experience:

High School diploma; or equivalent.

2 years experience and/or training in the field of Developmental Disabilities, psychology, etc. (May be waived by Associate or Executive Director.)

Valid California driver's license, current DMV record and maintaining proper insurance, if driving will be required.

Complete and pass Competency-Based Training classes provided by Harbor Supported Living Services in the first 90 days of employment.

LEAD STAFF JOB DESCRIPTION

Position: Lead Staff Home Supjport Specialist

Service: Supported Living Services provides support to adults with developmental disabilities who wish to receive support to live successfully in our community.

Purpose of position: The primary focus of the Lead Staff Home Support Specialist is:

1. Providing support to individuals in their homes and in the community by providing supervision, training, companionship, behavioral support, and other duties as deemed necessary by the Person receiving support and that Person's circle of support.

2. Crisis Support

3. Ensuring the safety of Person receiving supports, and other people, involved with SLS

4. Customer Service

Qualifications / Education: The successful Lead Staff is reliable, energetic, flexible, capable of working both independently and as part of a team, and is a positive role model for Supported Living's clientele. Qualifications include:

1. Satisfactory oral and written communication, math, and computer skills.

2. A high school diploma or its equivalent.

3. A minimum of one year of experience with adults who have developmental disabilities (or similar experience), is strongly preferred.

4. The ability to maintain First Aid and CPR certification.

5. Position may require a California driver's license, a good driving record, and a registered and insured vehicle.

6. Department of Justice, or criminal background check.

7. Familiarity with SLS programs and Person receiving supports is also beneficial.

Requirements:

1. Personal Assistance: Ability to assist the individual with activities of daily living including bathing, toileting, and other self-help skills.

2. Lifting / Physical Demands: Ability to lift 50 or more pounds frequently.

3. Challenging Behaviors: Ability and willingness to support individuals with challenging behavior.

4. Ability to: read and interpret documents, write specific reports and correspondence, apply mathematical concepts to practical situations, solve complex problems, and to interpret written and verbal instructions is required.

Status: Non-exempt, hourly position. Position may be full time or part time. If regularly scheduled for 30 or more hours each week, employee is full time and eligible for all applicable benefits.

Work Schedule: Hours dependent on Person receiving support's need. Staff must be available during business, evening, weekend, and holiday hours.

Duties / Responsibilities:

1. Creates monthly schedule and ensures shift coverage. Responsible for shift coverage if a substitute staff is not found.

2. Schedules monthly house meetings and ensure staff attendance.

3. Collects timecards, turns into office by deadline, bi-monthly.

4. Completes Monthly Service Reviews and Quarterly Safety Checklists.

5. Responsible for the review and upkeep of all household safety supplies.

6. Updates house binders at least once a month, insuring all paperwork (office forms, data collection forms, medication information, etc) is up to date and available.

7. Picks up medication, reviews for accuracy, creates and updates medication log as needed, at least monthly.

8. Posts monthly activity calendar, records all planned activities and meetings on house schedule, and posts any other Harbor correspondence.

9. Ensures Person receiving support safety in the home and community.

10. Assist Person receiving support in achieving personal, behavioral, educational, or vocational goals as identified in the Individual Service Plan.

11. Assists with some light household duties such as cleaning, cooking, and household management (may include yard upkeep) to maintain household in accordance to the guidelines determined by the Person receiving support and Person receiving support's circle of support. No significant amount of time should be designated to these tasks.

12. Provides necessary support for Person receiving support to meet personal needs.

13. Provides skills training when applicable.

14. Maintains current First Aid and CPR certification.

15. Participates in all mandatory trainings.

16. Responds to crisis in accordance with Harbor's policy and training

17. Assists Person receiving support with any medical needs or appointments.

18. Provides supervision and support to Person receiving support in the home and community.

19. Communicates and coordinates services with Facilitator and Harbor's management team.

20. Develops and maintains written documentation, as requested.

21. Complies with Harbor's goals, policies, and procedures.

22. Makes all decisions based on agency Mission and Values Statement.

23. Completes all other duties as assigned by supervisor.
