Location: RemoteThe Aftercare / Enrollment Coordinator is responsible for assisting with non-clinical duties for all phases of follow up after inpatient services. Primary duties may include, but are not limited to:Follows up on members who are identified by clinical operations as needing aftercare follow up.

Verifies compliance with initial follow-up appointments.

Provides assistance with referral to a provider if initial appointment is not attended.

Assists members in coordinating outpatient follow-up.

Checks provider availability.

Documents results of follow up calls in system.

Completes non-clinical assessment tools relating to after care or HEDIS activities.

Calls members and providers and gathers discharge and other non-clinical information from providers/facilities as scripts and workflow allow.

Develops and completes reports for completion of outstanding after care activities as needed internally and as requested by clients.

Assists and provides non-clinical activities related to HEDIS expectations as determined by quality management leadership.

Assists quality management staff with non-clinical work tasks as needed.

Qualifications Education: High school diploma or GED required, BA/BS in health related field preferred

Experience: 1 year experience in customer service and/or related field; or any combination of education and experience which would provide an equivalent background
