The Heart and Vascular Center at The Medical Center of Aurora focuses on the cardiovascular patient population, including arrhythmias, heart failure, ACS and valvular dysfunction.

Position Summary:

Under general supervision, in accordance with our mission and core values, utilizing advanced clinical knowledge and skills, the RN Coordinator will collaborate with physicians and nurse practitioners to provide direct care and oversee care coordination for patients in the Structural Heart Program both pre and post procedure, inpatient and coordinate the outpatient clinic. Prioritizing the patient experience and offering exceptional customer service are of upmost importance. Job duties include, but are not limited to:

  • Receive and review referrals for appropriateness, reviewing with an attending as needed.
  • Obtain all outside imaging, and other medical records
  • Coordinate initial work up and diagnostic testing for patients and families. Also assists families coming in from out of town in finding lodging and amenities.
  • Perform assessments and initial work up
  • Have all testing available for review by cardiologist and CTS at valve clinic visit
  • Participate in development of treatment or management plan. Assists in clinic alongside mid-levels and specialists
  • Patient and family education
  • Coordinates specialty visits
  • Facilitate direct contact between specialist and referring physician regarding treatment plan
  • Coordinate any additional testing i.e. cath, TEE, CT/MRI
  • Upload required data to appropriate vendor and maintain communication with device representatives Provide clinical knowledge for authorization of procedures/pharmaceuticals
  • Provide patients and families access to clinic remotely via phone
  • Participates follow-up visits and coordinates further follow-up
  • Initiates and coordinates outreach programs.
  • Initiates and coordinates education for hospital staff.
  • Tracks required data for quality
  • Attends weekly conferences where applicable
  • Enthusiasm for new treatment options for high and intermediate risk patients
  • May take call for patients needing off hours phone support
  • Consistently supports compliance and the principles of responsibility by maintaining confidentiality, protecting the assets of the organization, acting with integrity, reporting noncompliance, adhering to applicable federal and state laws and regulations and accreditation and licensure requirement
  • Other duties as assigned by supervisor.

Qualifications

Licensure/Certification/Registration: Current licensure in the State of Colorado as a Registered Nurse, or current active multistate Registered Nurse license. BLS required from AHA or American Red Cross.

Education: Graduate of an accredited school of nursing – BSN required, MSN preferred.

Experience: Background in cardiac or pulmonary preferred.

Special Qualifications: Strong focus on customer service and patient experience; ability to use Microsoft WORD, Excel, and data entry; ability to multi-task.
