The Residential Coordinator uses independent judgment and discretion to support current host home placements, family caregivers, and those in the supported apartment program. This position is responsible for the monitoring quality of services for and protection of placed adults, provides case management for individual adults, coordinates services for adults and their providers, and provides monitoring and support to those in residential services.

Duties and responsibilities

Completes home visits on a monthly basis to evaluate compliance with the residential inspection

Works closely with RN, LPN other members of team in developing safety and behavior plans for each adult

Provides ongoing training and resources to high-needs clients and families

Works closely in coordination with support coordinators, guardians, advocates, state personnel, therapists and families to monitor the adult in placement and ensure all needs are being advocated for appropriately.

Complete quarterly audits of the provider and client files, and ensures that all documentation is in compliance with CO Developmental Disabilities Standards

Respond to emergencies involving adults on caseload, and be available for rotation cycle for "on-call".

Requirements:

Qualifications

B.A/B.S in the social services preferred (Human Services, Human Development, Social Services, Psychology)

1-2 years of experience working with individuals with special needs and/or disabilities.

Valid driver's license, Insurance required, ability to pass a background check and UA.

Basic knowledge and understanding of behavior development and methods of crisis intervention.

Ability to communicate and interact with individuals from varying social, educational, and economic levels

Ability and willingness to work some hours outside of normal working hours to respond to emergencies.
