Faith based youth service agency seeks to fill the agency Case Manager position
General Purpose:
The Case Manager processes all intake and referral information to ensure TwoCor meets client census in the program. Communication to referral workers and stakeholders demand consistent and timely responsivity to numerous external and internal customers.
Essential Functions:
Case Manager receives referrals from partner agencies and facilitates all necessary communication and documentation for client enrollment, including regular visits to referring agencies for face to face interaction, engaging or facilitating other staff, with families of clients, with home visits, facilitating initial intake processes.
Case Manager co-facilitates orientation groups
Case Manager sends regular e-mail update reports to assigned individuals' case plan team
Minimum Education & Experience Requirements:
Bachelors' Degree
Previous experience with risk youth
Knowledge, Skills & Abilities Required:
Ability to operate Microsoft Office applications especially Excel
Desire to learn and apply trauma-informed principles in all tasks
Be detail orientated
Provide accurate and timely communication verbally and in writing
