What You’ll Be Doing:The Director, Health Equity creates new and innovation solutions for driving more equitable care delivery for members of a large health plans in Cook County. This is a problem-solving role that requires analyzing data, trends, and organizational capabilities to determine how to advance health equity for all members. This role will work to implement innovative solutions to dismantle racism and improve social risk factors for individuals that have historically been marginalized and/or facing socioeconomic, sociopolitical and environmental barriers to health - race, ethnicity, sexual orientation, gender identity, disability, and veteran status. The Director will design and implement solutions to health equity and will develop strategic plans and drive goals and objectives for improved performance.Responsibilities:

Analyze and understand current state of health disparities and key barriers in achieving health equity. Understand which populations are in the greatest need and build the strategy on how to address.

Design and implement health equity focused strategic plans, including tracking mechanisms and ROI designs in collaboration with CCH. Set targets and goals for the program.

Drive framework for analytics and research to identify health disparities in Cook County Health membership. Map all internal and external data sources that are available on health equity and create the metrics, benchmarks, and dashboard required to track progress.

Provide strategic guidance to HPS Population Health Team for health equity-focused pilots, value added-benefits / incentives, and clinical interventions

Align with members, providers and local community entities and support relationship management to drive organizational and process changes.

Lead cross functional workgroups to solution interventions to address identified gaps.

Implement and led the execution of agreed upon tactics and interventions.

Establish the governance model for prioritizing initiatives and tracking progress

Present data to create insightful and actionable business information to advance health equity and lead report outs to executive stakeholders, both internally and externally on ways to advance health equity.

Research best practices on advancing health equity and use proven solutions in proposed strategies.

Advance Evolent and HPS’s thought leadership based on evidence-based research.

Support development of “phase 2” of health equity dashboard and out year strategic plans.

Preferred Experience We Look For:

BA/BS degree and 4+ years of experience, preferably in public health, social/human services, social work, public policy, health care, education, community development, or justice; or any combination of education and experience, which would provide an equivalent background.

Deep understanding of social determinants of health, disparities impacting Cook County, and Medicaid populations

Experience with Provider Sponsored Health Plans highly preferred

Experience in working with State Medicaid Agencies required; experience and knowledge of IL Medicaid policies and procedures preferred

Knowledge of insurance industry standards

Possesses excellent problem-solving skills

Demonstrates through exceptional planning and organizational skills a record of bringing tasks and projects to successful conclusion

Exceptional communications and interpersonal skills

Demonstrates leadership skills with ability to work collaboratively in matrix environment to produce results across functional teams

Demonstrates ability to develop and execute action plans to meet goals and objectives

Demonstrates managerial and leadership abilities

Demonstrates strong analytical, organizational and critical thinking skills

Demonstrates ability to communicate clearly and concisely with Executive Management

Demonstrates skills in multi-tasking and prioritizing information and projects

Demonstrated excellent oral and written communication skills required

Located in Cook County or ability to travel to/and within Cook County as needed (10% - 50% in a post COVID-19 environment)

Ability to multi-task in a fast pace environment; can adapt quickly to change; have strong negotiations skills

Ability to conduct physician discussions and address escalations as needed

Technical Requirements:Currently, Evolent employees work remotely temporarily due to COVID-19. As such, we require that all employees have the following technical capability at their home: High speed internet over 10 Mbps and, specifically for all call center employees, the ability to plug in directly to the home internet router.
