DRCOG’s ADRC is hiring a Community Resource Specialist (CRS) who will have specialized knowledge of the public benefit system and how to navigate those systems. This position will support all consumers of the AAA department by exhibiting knowledge of those systems through telephonic work.

The successful candidate for this position will have in-depth knowledge of public benefits including SNAP, Medicaid, AnD, SSDI and SSI. The CRS will be able to support AAA consumers who are working with Extended I&A, Options Counseling, Case Management, and other programs by working directly with county technicians, monitoring for approval, changes or recertifications.

The CRS will be responsible for updating staff on new rules/processes for applications, developing relationships with the county Human Services offices and supporting consumers of AAA services of how to access public benefits. The CRS will also be providing support on the ADRC’s Information & Assistance line that provides support for older adults, adults with disabilities and their caregivers. The successful candidate will have strong time management skills and be able to work across different teams.

The work of the CRS is based on the foundational theories of Solution Focused Intervention within a strength-based service delivery mode and the ability to help guide a consumer toward their goals while engaging them in a change though is essential.

ABOUT THE POSITION OF Community Resource Specialist (Benefits Specialist)

This class specification represents only the core areas of responsibilities; specific position assignments will vary depending on the needs of the Division.

Performs client interviews, intake and assessments to determine service needs and resource requirements; serves as an agency representative with other service providers to address service requirements, availability and needs assessment; makes resource referrals to both internal and external programs; provides client advocacy based on required situations.

Evaluates and documents program eligibility status; determines case safety level, emergency status and/or need for crisis intervention or mandatory reporting; determines, develops, and communicates client service action/case plans; monitors and documents plan status and performs follow up on case services; evaluates plan outcomes.

Coordinates the exchange of service information and service care provision with other agencies; provides recommendations regarding course of action; assists in conflict or behavior management; and assists in the resolution of disagreements among clients, families, systems and service providers.

Monitors client status; provides counseling and intervention support; conducts on-site client visits and perform comprehensive needs assessment; performs safety and risk assessments; determines crisis intervention needs; monitors services and makes plan adjustments as client needs change; escalates case management supervision as required.

Prepares, documents and maintains case and/or program information; collects, manages and analyzes data to produce reports documenting program measures and outcomes; provides recommendations; ensures the maintenance of timely program or case activity tracking, reporting and applicable billing.

Provides training and community education for groups and individuals; researches and identifies community service needs; collaborates with stakeholders and makes program, process and community service recommendations; prepares and disseminates information; and performs outreach for care improvement. May be responsible for securing, monitoring and reporting special program funding.

Performs other duties of a similar nature and level as assigned.

Provide in depth knowledge of public benefits including Food Assistance, Medicaid, AND, SSDI and SSI to AAA consumers who are working with Extended I&A, Options Counseling, Case Management and other programs.

Be responsible for updating staff on new rules/processes for applications

Develop relationships with County Techs in the DRCOG region

Provide hands on assistance with completing new applications and recertifications to referred consumers

Provide follow-through and follow-ups at the County level to assure approval of benefits

Track and monitor applications for SSDI/SSI/SSA or refer to appropriate resources

Work in a call center environment taking calls from community members who need assistance to understand, access, and coordinate their transportation needs.

Work in tandem with staff to facilitate the full range of community-based service delivery options to older adults, care givers, and adults with disabilities to increase access to transportation services and other benefits and resource needs.

Conduct analysis of needs and provide coordination of services. Develop person-centered, individualized referrals, customized action plans, and client advocacy.

Attend meetings with community resource organizations to increase knowledge of services and allow for ‘warm transfers’ from clients to local services providers.

Collaborating with clients to ensure that they are meeting their goals and offering support to continue the path of self-sufficiency by providing encouragement and alternate resources and/or services.

Support the administrative needs of the program, including but not limited to, completion of mailing, data entry and billing, scanning and organization of program and client documents.

Provide support to Community Resource Team programs, staff and clients as needed.

Public benefits including SNAP, Medicaid, AnD, SSDI and SSI.

Education and experience requirements:

Bachelor’s degree in gerontology, social work or related human services field and one to three years of related human services experience. OR An equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above, unless otherwise subject to any other requirements set forth in law or regulation.

Necessary Special Requirements:

Certified Information & Referral Specialist for Aging/Disabilities (CIRS-A/D) certification.

Valid Colorado Driver’s License and access to an automobile or to substantively equivalent alternative transportation.

STATUS: Full-time, Non-exempt

The hiring salary range for this position is $4,114 - $4,610/month or $2,057 - $2,305/semi-monthly depending on qualifications.
