Seeking part time employee to work with adults with disabilities as a medical coordinator.

The medical coordinator is responsible for scheduling medical appointments for people served. The medical coordinator takes people to their appointments and works with the individuals team to ensure medical needs are met.

Community Intersections is excited to offer a hiring bonus. $100 after 30 days of employment and $100 after six months on the same payment schedule. The first half of the award amount will be received on the first available pay cycle after 30 days of employment and the second installment will be received upon the next available pay cycle after the candidate's 6-month anniversary if candidate remains employed.

The responsibilities of the Medical Coordinator include scheduling medical appointments for people served to meet their medical needs, taking people served to their appointments and work with treating providers to ensure the needs of the people served are met, providing transportation to and from appointments, and ensuring the consultation forms are completed and ensure all appointment results including order changes are communicated to other team members. This is a fun program and we always appreciate staff who are upbeat and optimistic. Although experience working with adults with developmental disabilities is preferred, we do provide all the necessary training for the direct care providers to support the people we serve. Applicants must pass a Criminal Background Check, Motor Vehicle record check, have a current license and a reliable vehicle. High school diploma/GED required unless participating in education assistance program described below.

Community Intersections is offering an education assistance program for employees who do not have their high school diploma or GED. Applicants must be enrolled in a GED or high school equivalency course at the time of start date and remain in program until attainment of high school equivalency to remain eligible for employment. Verification of enrollment must be provided on an ongoing basis. Bonus of $500 is paid upon successful completion. Employees receiving education assistance are not eligible for the hiring bonus described above.
