Unlicensed Clinicians: $5,000.00

based on the interconnection between physical, mental, emotional, and

spiritual health and approaches individuals with each of these components in

mind. Integrated Care allows for better continuity of care by improving

communication and care coordination between primary care and behavioral health

providers. Behavioral health services include assessments, therapy, crisis

management, and care coordination. This position will work in partnership

with Sunrise and Sunrise sites.Medication Assisted Treatment (MAT) is a collaborative approach to

treatment for opioid dependencies. Our MAT program works closely with Sunrise

Community Health and several other North Range Behavioral Health programs.Duties Performed:

Provide person-centered assessment, goal planning, and treatment for

mental health and/or substance use issues. Provide screening, intakes,

and assessment of clients utilizing required instruments and formats within

appropriate timelines.

Develop strength-based written treatment plans/updates and continuing care

plans and make appropriate referrals. Complete written discharge summaries and

correspondence within established timelines.

Observe clients in a variety of situations. Assess diagnosis and mental status

of clients, evaluate current level of functioning, and develop and

implement treatment/service plan to address clients' needs.

Intervene and de-escalate crises with clients. Provide crisis intervention via

telephone or face-to-face in a variety of settings that may include crisis

walk-in services, mobile on-site services, residential settings, etc.

Assess the risk of harm to self or others and arrange needed resources.

Collaborate with other clinicians, law enforcement, external providers,

and psychiatrists to provide services to clients. Maintain close and

cooperative rela ionships with community agencies and other public agencies in

carrying out treatment/service plans.

Collaborate continuously with assigned clients to devise safety planning,

provide symptom management, identify support systems, and reduce and

mitigate risk factors. May deliver services to clients in their homes or other

settings in the community.

Assist individuals in improving their well-being and quality of life by

training in behavioral skills; educate clients to make informed choices;

develop clients' coping skills; help clients to problem solve and

negotiate tasks of daily living.

Learn, utilize, and support fidelity of any evidence-based practices

within the assigned program to ensure effective client interventions.

Perform case management services as needed to assist clients to meet their

goals as stated in client service and/or treatment plans. Refer clients to

internal and external supportive services such as vocational, medical,

social services, and housing authority as well as residential, respite,

and inpatient treatment.

Maintain electronic health records following agency standards; confirm

coding of services rendered is ethically appropriate and results in successful

billing. This may include entry into a partnering agency's electronic systems.Education/Credentials:

A Master's Degree in psychology, social work, or another behavioral

health discipline from an accredited college or university is required.

A Valid Driver's License and driving record that meets agency insurability
