Description
- Essential Duties and Responsibilities (Other duties may be assigned)
- Analyzes and organizes home health operations for region of responsibility.
- Participates in the development of identifying future opportunities for home care specialization
- Monitors reports, metrics and progress towards goals established for the region and / or individual locations
- Serves as a resource to guide and direct regional service delivery.
- Represents the organization as a liaison with physicians and local community businesses.
- Communicates as needed with the Director of Nursing concerning new developments and/or problems.
- Responsible for review of Clinical Case Managers performance to include:
- starts of care
- timely re-certifications and authorizations
- adequate and appropriate supervision of clients
- effective billing controls
- care delivery and
- documentation requirements
- Personnel annual performance review and competency completion
- Assesses and assures Quality Improvement initiatives and any corrections and or Performance Improvement cycles are followed through
- Participates in the interviewing, hiring, and training of location leadership.
- Serves as an expert clinical resource to guide high quality training and competency assessments of care delivery staff.
- Stay current on regulatory rules for services delivered to guide compliant performance.
- Communicates and holds accountable local leadership for corporate projects or initiatives.
