Join our dynamic team! We are looking for a Care Coordinator who will be responsible for responding to needs presented by individuals and families who have been seen by the Connections Team for the assessment and referral process. Assists individuals/families in navigating and understanding the mental health and substance abuse system.
Required: Bachelors Degree in psychology, social work, family studies, or other human service related field. At least two years of experience working in the behavioral health field.
Preferred: Bilingual skills written and oral (English/Spanish). A Masters Degree in an appropriate discipline.
