Position Title: Adult Services Case Manager

FLSA Status: Full Time Non Exempt

Required Travel: Typically Local, High 50-90%

Salary: Hiring Range: $18.11 to $19.74 per hour DOE

Position Summary

The ASA Case Manager is responsible for providing person-centered care coordination for individuals with developmental and intellectual disabilities to promote a high quality of life.

Essential Duties and Responsibilities

1. Establish, build, and maintain rapport with individuals, independent contractors, and other member of the multidisciplinary team in order that optimal information gathering, and collective decision making may occur. These duties must be carried out in a manner that respects the rights and dignity of individuals, is driven by individual choice and promotes community inclusion.

2. Provide case management for individuals. Work closely with, and act as the liaison between collaborating agencies/multi-disciplinary team.

3. Develop, monitor and document mandated services and programming, and ensure client satisfaction. Ensure individual files are audit ready and in compliance with Volume 8 rules and regulations.

4. Understand and participate in all relevant assessments to ensure appropriate level of care for individuals.

5. Facilitate home visits and meetings to assess the needs and progress of individuals and intervene as appropriate. Provide mediation as needed.

6. In a timely manner, effectively and appropriately communicate potential risks and/or threats to an individual or program to Ariel’s supervisors and collaborating agencies.

7. Participate in mandatory rotation for “on call” to support our clients.

8. Participate in ongoing program evaluation to ensure program effectiveness.

Education Requirements

Required:

Bachelor’s degree in social or behavioral sciences, or a combination of four years’ experience and formal education in a Human Services field.

Preferred:

Bilingual/Bi-literate in Spanish or American Sign Language.

Minimum of 2 years working with I/DD population.

1 yr. mental health experience.

Certifications and Licenses

Valid Regular Driver’s License, with acceptable driving record.

CPR, 1st Aid – before contact with clients.

Safety Care – within 60 days of hire.

Informed Supervision –deviant sexual behaviors training - within 60 days of hire, may not be left unsupervised with clients until this training is completed.

QMAP – (Medical Administration Certification) within 60 days of hire, may not provide medication to clients without this certification.

Client Specific Training of individuals on caseload.
