Position Title: Adult Services Case Manager

FLSA Status: Full Time Non Exempt

Required Travel: Typically Local, High 50-90%

Salary: DOE

Position Summary

Under the direction of the ASA Program Manager, the ASA Case Manager is responsible for the case coordination of Ariel Clinical Services Developmentally Disabled Clients. This includes functioning as a member of a multi-disciplinary team to provide: clinical evaluation and treatment of clients, their host homes, families and outside agencies. To provide direction necessary to enable persons with developmental disabilities to maintain skills that will foster personal growth and independence. These duties must be carried out in a manner that respects the rights of individuals, is driven by individual choice and based on the concept of community inclusion.

Education Requirements

Bachelor's degree in social or behavioral sciences preferred or a combination of five years experience and education in the field of developmental disabilities.

Certifications and Licenses

Required

1. Valid Regular Drivers License, with acceptable driving record.

2. CPR, 1st Aid, CPI, SIP, QMAP, etc as outlined by supervisor.
