Bilingual Licensed Practical Nurse (Haitian Creole and Spanish)Job DescriptionWe are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Primary Care team. This role focuses on high-risk and chronic care management support, referral specialist support, and improving compliance with treatment plans. We provide primary healthcare, dental, and mental health services to children and adults living in Greenwich and Stamford, with an emphasis on the special population living in or near public housing.Responsibilities
Provide routine nursing care to patients.
Support high-risk and chronic care management.
Assist in care coordination for elderly and disabled patients, as well as those with chronic conditions such as diabetes, HIV, hypertension, and obesity.
Manage and improve compliance with treatment plans.
Support referral processes and care coordination.
Travel between Stamford and Greenwich as required.
Essential Skills
Current LPN licensure in CT required.
Minimum of 2 years of experience in a primary care or outpatient setting preferred.
Experience in high-risk or chronic care management preferred.
Knowledge of population health, care coordination, and referral processes.
Proficiency with electronic health records (EHR).
Excellent communication and interpersonal skills.
Ability to work collaboratively within a multidisciplinary team.
Bilingual required – Haitian Creole preferred.
3+ years of experience working as an LPN.

Pay and BenefitsThe pay range for this position is $33.00 - $36.00Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
