Home Care Registered Nurse (RN)

Job Description

We are excited to announce a new opportunity to work in our home care division and see patients in the Lower Fairfield County area. Our organization is rapidly expanding our treatment services and there is plenty of room for growth potential! As the home care registered Nurse, you would work alongside an established team in a collaborative work environment. Please see job details below for more info.

• Seeing patients in their homes and assessing physical condition
• Meeting with patient family members
• Documenting notes in EMR system
• Collecting important information and advising patient on safety procedures
• Participating in team meetings
• Providing ongoing nursing treatment

• Associates Degree of Nursing or BSN
• Preferred home care experience
• Active Connecticut driver’s license
• Active BLS
