This part time visiting associate plays an integral part in this process by serving as the liaison for House Calls visits.ResponsibilitiesThe successful team member will work in different work settings including patient homes.Comfortable working in different environments

Must be able to follow policies and procedures and work flexible schedule and location.
Job dutiesCall patients in anticipation of the home visit.
Collects Personal Protective Equipment from base center.
Ensure that phone and iPad are in working order.
Drive to and from patients' homes for visits in a timely manner.
Setup the iPad and tripod to facilitate the visit between the patient and Primary Care Physician.
Take note of the following:
Condition of the patient
Condition of the patient's surroundings
Determine if the patient has access to food
Report findings to Supervisor.
Submit mileage in a timely manner.
Other duties as assigned
Required QualificationsFully bi-lingual English and Spanish
Excellent time management skills
Able to manage technology well
Valid driver license and dependable transportation necessary
This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
We will require full COVID vaccination (https://www.cdc.gov/coronavirus/2019-ncov/vaccines/fully-vaccinated.html#vaccinated) for this job as we are a healthcare company committed to putting health and safety first for our members, patients, associates and the communities we serve. This job is visiting patients and requires close personal interaction with patients. You will not be able to social distance.
If progressed to offer, you will be required to provide proof of full vaccination where allowed by law.
Preferred QualificationsHigh School Diploma
Microsoft Access proficiency
