Professional Case Management Certified Nurse Aides or Home Health Aides work in a one-on-one setting in a client’s home, under the direct supervision of a Registered Nurse/Case Manager. Working in this environment gives our caregivers the opportunity to provide a high level of individualized care while helping our clients live at home.

As a CNA/HHA with Professional Case Management, you will provide professional and compassionate in-home care to former Department of Energy workers who are suffering from chronic illnesses due to work environment exposure. Your duties may include but are not limited to:

Reading and recording temperature, pulse, and respiration

Basic infection control procedures

Maintenance of a clean, safe, and healthy environment

Safe transfer techniques and ambulation

Personal Hygiene

Meal preparation and feeding including adequate nutrition and fluid intake

Provide daily reminders of medications

Professional Case Management understands the importance of a work/life balance and offers flexible scheduling to work with most employees schedule needs. Most shifts at PCM are 8-12 hour shifts but we occasionally have shorter shifts available.

Professional Case Management directly hires all employees and provides workers compensation and liability insurance for all employees.

Requirements

At least eighteen (18) years of age

Successful completion of a formal aide certification training program consistent with state and federal requirements and for which a certificate was obtained (Colorado: certified within four months of starting employment and certificate remains current TN: completed a total of seventy-five (75) hours of training which included sixteen (16) hours of clinical training prior to or during the first three (3) months of employment)

Successful demonstration of competency in the provision of care and services safely and effectively

Minimum of six (6) months work experience in a supervised setting, preferably a health care facility or home health agency; prefer one year experience

CPR certification

Demonstrated ability to read and write consistent with job requirements

Verbal, written and interpersonal skills necessary for establishing and maintaining a good working relationship with the client/family and personnel of the Agency

Licensed driver with good driving record and automobile in good repair and insured in accordance with state and/or Agency requirements or other reliable means of transportation
