Hale O Meleana in Honolulu, Hawaii is looking for a Part-time Medication Aide. Under the direct supervision of the Resident Care Coordinator/Licensed Nurse and indirect supervision of the Director of Nursing, the Medication Assistant is responsible for providing healthcare services via safely administering medications and treatments to residents and on-going consultation under the supervision of a Registered Nurse of Licensed Practical Nurse. Additionally, Medication Assistants provide care and services for residents that include, but is not limited to, personal care and activities of daily living as needed. Our team, works to promote, maintain, and encourage all residents to function at their highest physical, mental, and psychosocial level. Medication Assistants are required to perform job duties in accordance with Ohana Pacific Health (OPH) Core Values and behaviors.

Shift: All shifts

Duties include but are not limited: 

Administers the medications and treatments, according to the scheduled times, following the medication administration protocol.

Observes residents for drug reactions and report any problems to the nurses.

Performs tasks associated with medication administration, including resident vital signs, calculate medication dosages, and stock the medication cart.

Records medication administration according to federal, state, local, and Company guidelines. All records and documentation must be legible and accurate on the medication record MAR.

Faxes medication orders to pharmacy; assists with re-orders.

Notifies physician when pre-authorizations are required, or medications not covered by insurance.

Monitors medication when it arrives from the pharmacy and reports discrepancies to pharmacy.

Accurately completes a medication error report if necessary

Maintains the cleanliness and organization of the med room and medication cart

Inputs and edits medication orders in EMR under supervision of the RCC/LN. Updates responsible parties/family on new or changes in orders by providers.

Responsible for fulfilling daily, weekly, and monthly job checklist duties.

Accurately and completely documents information related to resident:

Record food and fluid intakes; Record weight, vital sign and showers; record ADL's on ADL sheet or in electronic system.

Accurately completes an incident report and begins the investigation

Assist resident with transportation and appointments as needed.

Responds to resident calls, call pendant call lights, alarms immediately. Responds to family requests immediately. Informs RCC/LN when required.

Assists in resident admissions and orients residents to the apartment and community.

Assists with discharging of a resident.

Meals and nutritional responsibilities such as assists residents to designated table for meals/snacks, checks meal to ensure correct preferences, diet and modifications are served, serves residents and assist as needed, and assist with dining room service (set up, take orders, serve, bus tables), including but not limited to:

Assists residents to maintain highest level of functioning such as encourage use of devices that promote vision, hearing, eating and mobility, provide transfer assistance with staff, gait belts, bed rails, walker, mechanical lift, provide ambulation assistance with gait belt, staff, cane, walker, parallel bars.

Appropriately monitors resident conditions and reports to RCC/LN in a timely manner.

Maintains confidentiality of resident information and the business of OPMC and Seasons Living.

Requirements:

Minimum of 6 months of experience in long term care, healthcare, or work with the elderly preferred. Good command of English, both written and verbal. Must possess the verbal ability to communicate tactfully with guests, caregivers, visitors, physicians, government agencies / personnel and the general public. Able to meet the physical and emotional demands of providing resident care.

High school diploma or equivalent.

Satisfactory completion of a Nurse's Aide Program.

State of Hawaii Board of Nursing Certification for Nurse Aide preferred.

First Aid and CPR preferred. (can be done in-house).

Must have current medical/health screening including 2 step TB clearance (can be done in-house).
