The Arc in Hawaii is a non-profit organization with 70 years of history in Hawaii. We provide both residential and day services for adults with intellectual and developmental disabilities (IDD). Our mission is to support the inclusion and participation of individuals with IDD in their respective communities throughout their lifetime.

Role:

We are seeking a Nurse Case Manager to provide Arc clients (adults with IDD) with necessary services to attain and maintain optimal health status. You will be responsible for ensuring access to and the provision of quality medical care to our clients (participants/residents). This position requires minimal hands-on nursing skills and focuses more on educating and training direct care staff on clients’ Health Maintenance Plans, medication administration, infection control, skin assessments, feeding guidelines, fall prevention, seizure precautions, and daily monitoring.

Responsibilities: (include but are not limited to)

Ensures that the nursing standard operating policies and procedures are current and updated, and staff members are trained in said policies and procedures.

Provides basic FA/CPR to participants, residents, staff, and others when needed.

Provides health care explanations and guidance to families and guardians.

Maintains accurate and complete documentation in client medical records by all individuals providing health care; provides reports regarding medical concerns when necessary.

Observes and assesses clients’ condition through monthly and annual exams and provides direct care whenever necessary.

Assesses and identifies clients’ needs, determines plan of action, implements plan, re-evaluates plan to ensure effectiveness and completeness of approaches.

Communicates with other disciplines regarding clients’ specific needs. Participates in Individualized Program Plan conferences and documents clients’ progress in T-logs.

Coordinates provision of care by primary care physician, specialists, and other health care personnel, and attends clients’ medical appointments and exams as assigned.

Conducts and documents staff training on plans of care, medication administration, nutrition, infection control, and unique client-specific medical needs.

Performs and documents site reviews of client health, safety, medication administration, and infection control as scheduled and as needed.

This role sometimes involves driving to our homes/centers, etc. to work with our clients, staff, etc.

Requirements:

Associate degree or higher in the field of nursing from an accredited college or university

Current Hawaii RN license

Valid driver’s license and reliable transportation (and clean driver’s abstract)

Smartphone

1 year experience as a registered nurse

Able to communicate clearly in English, both verbally and in writing

Good interpersonal skills, positive, helpful, patient, good judgment

Able to multitask, stay organized, prioritize work, and meet deadlines

Proficient in computer software programs related to job duties

Able to operate standard office machines, standard household appliances, and assistive devices including Hoyer lift, wheelchair, walker, and gait belt

Able to work a flexible work shift, possibly including weekends, evenings, overnights, and holidays, if needed. Able to respond to emergency calls on a 24-hour, 7-day per week basis.

Work Clearances (Conditions of Employment):

Pre-employment drug test

Physical exam

TB test (initial 2-step PPD, annual 1-step PPD)

CPR and First Aid/AED certification

Criminal history check

Adult Protective Services (APS) and Child Abuse & Neglect (CAN) registries

FBI fingerprinting

Valid driver's license and clean driver’s abstract (or acceptable level of infractions)

Must remain current on all work clearances

Must attend all required trainings (paid)

Drug test, FBI fingerprinting/APS/CAN, physical exam, and FA/CPR/AED certification are paid for by The Arc in Hawaii.

Preferred Qualifications:

1 year volunteer or work experience assisting persons with intellectual or developmental (ID/DD) disabilities

Salary: $55,000 to $65,000 per year
